Impact of Group Medical Visits on Patient Engagement and Quality of Life

Group medical visits (GMV) are an evidence-based care model bringing patients with a mutual chronic condition together for longer billable visits in a group setting. GMVs are led by providers trained in facilitating discussion. The team of facilitators direct the conversation and allow patients to learn from each other as well as the health care team, empowering participants to take ownership of their health and support one another. While there is proven benefit for the GMV model in primary care, contributors to patient outcomes need further investigation. Understanding a patient’s level of commitment to engage in their health management is key for long-term success. We have established a GMV in our clinic in the areas of diabetes, healthy lifestyle education, centering parenting, and tobacco cessation. The objective of this study is to determine if patients who participate in GMV score higher on the Patient Activation Measure (PAM) and SF-12 (to measure health-related quality of life, or HRQOL). Data from baseline to group visit completion (6-8 visit series) and 6-month follow-up will be evaluated and predictive of health outcomes. Our preliminary results of 105 patients indicate acceptance of the GMV model and improvement of scores over time. Challenges with GMV follow-up with COVID-19 restrictions have impacted follow-up. Implementing a measure of patient engagement and HRQOL assessment are useful to evaluate patient health outcomes.
