Over view of development of an Interprofessional Curriculum in Geriatrics and Palliative care at a Patient Centered Medical Home Residency Clinic. The goal of the clinic was to provide learners with a shared didactic and clinical experience to further support their competencies and expected milestones in geriatrics and palliative care while providing the best patient focused care.Resources include overview of development, the competencies by profession and tools used to teach these competencies, Resident IPT facilitationo guide, Resident self evaluation form and other evaluation forms including New Innovations Milestones evaluation of resident, faculty shadow evaluation form, and patient clinic evaluation form. Evaluations and curriculum review are used for continued practice improvement.
