Michigan Medicine has participated in the CPC+ program for the past 2 years. Over time, we have modified clinical workflows and reporting in order to meet the program requirements. Although our system has had experience with other comprehensive payment demonstration projects, participation in CPC+ has led to the need for new capabilities and system-level changes. This session will review the state of primary care at the beginning of participation, and outline the journey we have undertaken to meet the requirements over time.
