This is the study questionnaire used to assess physicians’ knowledge, adherence to clinical guidelines, and physicians’ suspicion of eating disorders (ED) or binge eating disorder (BED). To assess physicians’ knowledge, medical knowledge questions were derived from the diagnostic criteria in the DSM V-TR. To assess physicians’ adherence to clinical guidelines, questions were generated from USPSTF and ACOG clinical guidelines. To assess physicians’ suspicion of ED or BED, questions were generated to assess physicians’ approach to weight based on patient’s BMI.
