01-22-2017 23:13

Conference on Medical Student Education 2017 Presentation: This session describes the launch of an educational pilot program that engaged medical students in the care of homeless patients as part of an interprofessional team. Upon completion of this session, participants should be able to understand how an IPE pilot can support the following objectives: Apply interprofessional collaborative competencies to improve outcomes for vulnerable populations and patients living with diabetes, mental health issues, and other chronic diseases. Enhance knowledge, skills, experience, values, and behaviors previously acquired in interprofessional team learning and apply evidence-based team communication tools to community health settings. Translate interprofessional strategies learned into practice to assess, manage, and consult in the care of complex patients with multiple social, cultural, and health co-morbidities in a community setting.
