Behavioral Health Collaborative Care: The Michigan Medicine Model for Integration

Michigan Medicine initially started to develop a model for integrated behavioral health care in 2014. Using the Washington Behavioral Health Integration Program as a model, a team composed of primary care physicians, psychiatrists, and social workers developed a comprehensive program for management of patients with mild to moderate depression and anxiety. The program was piloted at two initial sites in family medicine and general internal medicine. It is now in the process of being implemented across all of primary care. This session will review the model and share experiences from implementation at one family medicine site.
